Due to medical regulations, as an FDA approved facility, we are unable to accept returned medical devices that have been opened.

 

Warranty

This Warranty applies only to Provale Cups purchased directly from ProvaMed or an approved distributor.

ProvaMed warrants this product against defects in workmanship or material for a period of 60 days after purchase if the product was purchased directly from the Company or an approved distributor. Defective product may be returned to ProvaMed for repair or replacement by contacting the Company. The returned product, prepaid transportation charges, together with a copy of the original invoice or sales receipt are required.

Other than product being returned for workmanship or material defects, product that has been opened from the individual cup box can NOT be returned due to FDA regulations.

Damage caused by accident, misuse, or abuse shall not be covered by this warranty. This warranty covers parts, but does not cover shipping costs or any incidental or consequential damages. It is the responsibility of the buyer to ensure that this product is used in a manner which is appropriate for its intended use.

This warranty is in lieu of all other warranties, expressed or implied, and no one is authorized to assume for ProvaMed. any other liability in connection with the sale of this product. This warranty gives you specific legal rights. You may also have other rights, which vary from state to state.
